29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15-, 20-, and 30-ampere receptacle outlets that were not part of the permanent wiring of the building or structure and that were in use by personnel did not have ground-fault circuit-interrupter protection for personnel. On or about August 11, 2021, in the mud mixing area above the sink there was an electrical receptacle (four outlet) that was not GFCI, this condition exposed employees to electrical shock hazards .
