Plant #10 - On August 17, 2021 an employee was seriously injured when his leg was crushed between the steel baseplate of a concrete form that was being lifted by an overhead hoist and a concrete manhole structure. The employee was standing on the baseplate as it was being hoisted, the plate shifted and he fell between the load and the concrete structure, pinning his leg.
