OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to serious injuries associated with the improper operation of underhung hoists: a. Plant #10 - On August 17, 2021 an employee was seriously injured when his leg was crushed between the steel baseplate of a concrete form that was being lifted by an overhead hoist and a concrete manhole structure. The employee was standing on the baseplate as it was being hoisted, the plate shifted and he fell between the load and the concrete structure, pinning his leg.
