29 CFR 1926.754(e)(5)(i): Metal decking was not laid tightly and immediately secured upon placement to prevent accidental movement or displacement: (a) Construction site - On or about August 16, 2021, the metal deck panels on the roof used as a walkway and as the leading edge were not secured to prevent displacement, exposing employees to a 27-foot fall hazard.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. (a) Construction site - On or about August 16, 2021, employees installing the metal deck panels on the roof and while working at the leading edge, were not protected from a 27-foot fall hazard by the use of a fall protection system.
