On or about 8/17/2021, at 322 North A Street, Farmington, MO 63460, the employer did not ensure that each employee on a 12:12 pitch roof, approximately 22 feet above ground level, was protected from falling. The employer failed to protect their employees by not providing one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems.

The ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.
