This was most recently documented on or about August 17th, 2021 in Omaha, Nebraska. Employees are using damaged extension cords that had apparent damage to the sheathing of the extension cord. The employer used tape to cover the the sheathing of the extension cord - These extension cords were uses to supply power to equipment at the worksite.

29 CFR 1926.451(c)(2)(v):Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied. The employer is failing to protect employees from fall hazards. This was most recently documented on or about August 17th, 2021 in Omaha , Nebraska. An employee was observed working on a platform that wasn't secured to the forklift - The approximate height the employee was working at was over 6ft.
