29 CFR 1926.403(b)(2): Installation and use. Listed, labeled, or certified equipment were not installed and used in accordance with instructions included in the listing, labeling, or certification. On or about August 09, 2021, in the Dining Area; several extension cords were daisy chained to one another. Employees were exposed to electrical shock hazards.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. On or about August 09, 2021, at the Reeves County Hospital Replacement Hospital in the dining area the electrical cord had a busted strain relief. The 120 volt fan cord exposed employees to electrical / shock hazards.
