29 CFR  1926.251(b)(6)(i):In addition to inspections required by other paragraphs of this section, a thorough periodic inspection of alloy steel chain slings in use was not made on a regular basis at intervals no greater than once every 12 months.  a) Jobsite: 2508 E Cumberland St Philadelphia, PA - On or about July 26, 2021, the employer did not conduct periodic inspections of their alloy steel chains used to rig an I-Beam on the basis of frequency of use, severity of service conditions, nature of lifts made, or service life, at intervals not greater than every 12 months.

29 CFR  1926.20(b)(1):The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) Jobsite: 2508 E Cumberland St, Philadelphia, PA - On or about July 26, 2021, the employer did not develop and implement an accident prevention program to assist in identifying working conditions and hazards in concrete foundation operations to include, but not limited to struck-by, caught-between, electrical hazards, etc. related to lifting loads using earth moving equipment.
