Employees were working from the second level frame scaffold platforms that were not fully planked, thereby exposing employees to fall hazards of approximately 12 feet.

Employees were performing masonry activities from a supported frame scaffold that did not have guardrails on the sides of the working platform, thereby exposing employees to fall hazards of approximately 12 feet.

Employees conducting masonry activities utilized a portable ladder to access a scaffold. The ladder did not extend at least 3 feet above the scaffold platform, thereby exposing employees to fall hazards of approximately 12 feet.
