29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety. a) W Preston Ave, Water Treatment Plant, Thompson Falls MT: On or about August 18, 2021, and at times prior, employees were exposed to cave in hazards while removing Lite Forms from concrete in an unguarded excavation with vertical walls between soil and a concrete form at depths of approximately 10 feet. The competent person failed to remove the employees from the excavation in situations that could result in a cave in.
