29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Jobsite located at 390 Boot Rd, West Chester, PA: In the Mechanical Room of Bldg. 104 (Learning Center), the Trane Climate Changer Air Handler, serial no. K92C13894, did not have guards installed for a protruding rotating shaft at the sleeve bearing access point in front of the unit & a rotating belt assembly at the back of the unit, exposing employees to caught-in-between hazards. Condition noted on or about August 17, 2021.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures: (a) Jobsite located at 390 Boot Rd, West Chester, PA: Employees performing work inside the sprinkler room of Bldg. 104 (Learning Center) were exposed to electric shock hazards in that condensation from an overhead water pipe was dripping down and accumulating onto the electrical control panel of a Weil-McLain natural gas boiler, model no. LGB-11. Condition noted on or about the 6-month period of Feb 17 - August 17, 2021.
