29 CFR  1910.502(c)(1): The employer must develop and implement a COVID-19 plan for each workplace.  If the employer has multiple workplaces that are substantially similar, its COVID-19 plan may be developed by workplace type rather than by individual workplace so long as all required site-specific information is included in the plan.     a) SimonMed Imaging, Inc. - Beltway located at 11375 South Sam Houston Parkway West, Ste. 150, Houston, TX 77031:  On or about August 12, 2021, the employer failed to implement their COVID-19 plan to mitigate the spread of SARS-CoV-2, the virus that causes Coronavirus Disease 2019 (COVID-19).  The employer did permit the implementation of the site-specific procedures developed for the screening of suspected and confirmed COVID-19 patients.
