29 CFR 1926.1053(b)(15):Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use: On or about August 18, 2021, the employer failed to ensure that ladders being used in the workplace, to include but not limited to the blue Werner 8 foot fiberglass, were inspected prior to use to ensure there were no defects and that all labels were identifiable and readable to ensure proper safety when in use.

29 CFR 1904.31(a):Recordable injuries and illnesses of all employees on your payroll, whether they are labor, executive, hourly, salary, part time, seasonal, migrant workers, and those employees who are not on your payroll, but supervised by you on a day-to-day basis were not recorded on the OSHA 300 Log.: On or about August 18, 2021, the employer failed to produce upon request, the OSHA 300 and 300A logs for the years 2016-2021. The employer did not maintain any logs during this time period.
