29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about June 11, 2021, employees conducting roofing installation operations on the upper level of a residential single-family property were exposed to fall hazards of approximately 11.8' when the employer failed to ensure that fall protection was utilized.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On or about June 11, 2021, the employer failed to report a hospitalization of an employee who was hospitalized after falling from the upper level of a single family residential property.   Abatement certification is required of this item in accordance with requirements of 29 CFR 1903.19(c).
