29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 41 Jennings Drive, Albany, NY - On or about August 2, 2021, an employee was injured and sustained an amputation to his left index finger just below the nail. The employer failed to report the amputation to OSHA within the 24 hour time frame.
