29 CFR  1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with a stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements.   a. On or about August 19, 2021, at the southwest corner of the main building, the employer did not ensure that each flight of the 54 inches wide, finished, permanent and currently in use, metal pan treads, stairs connecting the first and second levels, having 4 or more risers, was equipped with a stair rail system and handrails on each enclosed side as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements.  NOTE:  Certification of abatement is not required for this citation item since it was submitted prior to citation.

29 CFR 1910.305(b)(1)(ii):  Unused openings in electrical boxes, cabinets, or fittings were not effectively closed.  a. On or about August 19, 2021, at the RBC Construction Corporation onsite office trailer, the electrical service panel had three unused circuit breakers openings, exposing the energized bus bar. Employees were exposed to electrical hazards from an open electrical service panel with exposed live parts through unused openings.  NOTE:  Certification of abatement is not required for this citation item since it was submitted prior to citation.

29 CFR  1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing or labeling.  a. On or about August 19, 2021, at the RBC Construction Corporation onsite office trailer, an extension cord, powering the coffee maker and mini fridge, was serially connected to a Relocatable Power Tap instead of directly to a branch circuit receptacle; exposing employees to electrical and fire hazards.  NOTE: Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.
