29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) T.A.C Brothers Services LLC at 200 Fox Run Lane in Fraser, CO 80442: On or before August 20, 2021, the employer did not ensure that employees working at heights to replace shingles on a residential building roof used fall protection equipment. This condition exposed employees to fall hazards of approximately 20 feet.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) T.A.C Brothers Services LLC at 200 Fox Run Lane in Fraser, CO 80442: On or before August 20, 2021, the employer did not ensure that employees working at heights to replace shingles on a residential building roof were properly trained in the nature of the fall hazards in the work area and how to recognize them, the procedures to be followed to minimize those hazards, the correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses, and the correct use of personal fall protection systems and equipment. This exposed employees to falls of approximately 20 feet.
