29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) C & E Roofing and Exteriors LLC at 92 Doc Susie Ave in Fraser, CO 80442: On or before August 20, 2021, an employee engaged in roofing support was working below other employees working two-stories above without head protection and was exposed to struck-by hazards from falling objects.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) C & E Roofing and Exteriors LLC at 92 Doc Susie Ave in Fraser, CO 80442: On or before August 20, 2021, the side rails of the extension ladder employees used to gain access to the roof did not extend at least 3 feet above the landing surface. This exposed employees to a fall hazard.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  (a) C & E Roofing and Exteriors LLC at 92 Doc Susie Ave in Fraser, CO 80442: On or before August 20, 2021, the employer did not ensure that employees working at heights to re-roof a two-story residential building used fall protection equipment. This condition exposed employees to fall hazards of greater than 6 feet.  C & E Roofing and Exteriors LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1306395, Citation 1 Item 1, and was affirmed as a final order on July 3, 2018, with respect to a workplace located at 8328 Mummy Range Drive, Fort Collins, CO 80521.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) C & E Roofing and Exteriors LLC at 92 Doc Susie Ave in Fraser, CO 80442: On or before August 20, 2021, the employer did not ensure that employees working at heights to re-roof a two-story residential building were trained on fall protection. This condition prevented employees from recognizing and mitigating exposure to fall hazards of greater than 6 feet.
