29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: New townhome construction located on Lot #95 Blackburn Court, Smyrna, GA 30080, front side: On or about 08/13/2021, the employer exposed employees to collapse/ fall hazards of approximately 14-feet, 4-inches to the lower level, in that the scaffold support poles were not resting on a mud sill, base plate or other adequate foundation as the supports were also not secured to prevent movement during use.

29 CFR 1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. New townhome construction located on Lot #95 Blackburn Court, Smyrna, GA 30080, front side: On or about 08/13/2021, the employer exposed employees to fall hazards of approximately 14-feet, 4-inches to the lower level, in that a fall protection system was not used by an employee installing siding as he was not attached to the life-line provided on the roof.
