29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about August 20, 2021, an employee was applying shingles to a roof deck with a pneumatic nail gun without the use of eye protection, exposing the employee to the hazard of being struck to the face and eye(s) by flying particles or projectiles.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   On or about August 20, 2021, an employee was laying shingles on a roof with unprotected edges without fall protection, exposing the employee to the hazard of falling at heights of 6-feet or greater.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access.  On or about August 20, 2021, an employee using a portable ladder to access the roof of a residential home under construction did not ensure the ladder side rails extended at least 3 feet above the roof deck edge before access was gained, exposing the employee to the hazard of falling from the ladder at approximately 10 feet.
