29 CFR  1926.451(f)(3):Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  Location: NY Rt. 306 & Kearsing Parkway Monsey,  NY 10952  a) On or about August 25th, 2021, pump jack scaffolds in front and back of building "3" were not inspected by competent person before use due to employees working on scaffold that did not have guardrail system, secure and leveled platform, secured and level posts, and correct mudsill plates installed. Employees were exposed to fall hazards.

The employees used a window from the 3rd floor as access to the scaffold.
