(a) Delivery Site, Baron Blakeslee Manufacturing Corp, 200 Armstrong Avenue, Williamston, WV: On August 23, 2021 and times prior, employees would operate a 2014 Dodge ProMaster, 2017 Dodge ProMaster or 2013 Dodge Ram to make deliveries and leave the vehicle unattended without first engaging the parking brake, exposing them to struck by hazards.
