On or about August 24, 2021, during the installation of an inflatable Cow on the roof of a shipping container located in the parking lot across the street from 8213 Golden Link Blvd, Macedonia, OH 44067; Employees were observed standing on the roof of a shipping container while emptying the components of a storage tub and rolling out a deflated cow without the use of PFAS or protected by any other form of fall protection system. The roof height was approximately 8.3 feet above the ground.
