29 CFR  1910.23(c)(11): Portable ladders used to gain access to an upper landing surface have side rails that extend at least 3 feet (0.9 m) above the upper landing surface.   On or about August 24, 2021, employees gaining access to the upper landing surface of a shipping container were using a ladder that did not have side rails that extended 3 feet above the upper landing surface located in the parking lot across the street from 8213 Golden Link Blvd, Macedonia, OH 44067; Employees were using a step ladder to gain access to the roof of a shipping container.

29 CFR  1910.28(b)(1)(i):(1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling .  On or about August 24, 2021, during the installation of an inflatable Cow on the roof of a shipping container located in the parking lot across the street from 8213 Golden Link Blvd, Macedonia, OH 44067; Employees were observed standing on the roof of a shipping container while emptying the components of a storage tub and rolling out a deflated cow without the use of PFAS or protected by any other form of fall protection system. The roof height was approximately 8.3 feet above the ground.
