29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a) Upper Warehouse, Upper Mezzanine - Employees working from the upper mezzanine were exposed to a fall hazard of approximately ten and a half (10.5) feet above a lower level without being protected by a guardrail system, safety net system, or personal fall arrest system, on or about August 25, 2021.  (b) Lower Warehouse, Upper Mezzanine - Employees working from the upper mezzanine were exposed to a fall hazard of approximately twenty (20) feet above a lower level without being protected by a guardrail system, safety net system, or personal fall arrest system, on or about August 25, 2021.  Abatement Certification not Required.

29 CFR  1910.36(g)(2): Exit accesses were not at least 28 inches (71.1 cm) wide at all points:   (a) Pallet Racking, Upper Warehouse - Employees working in the pallet racking area of the upper warehouse were exposed to fire hazards in that product storage restricted the exit access to be less than twenty-eight (28) inches wide, on or about August 25, 2021.
