29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about August 19, 2021, and times prior thereto,  employees climbed over the railing of a scissor lift in order to access the roof of a building under construction, exposing the employees to the hazard of falling approximately 17 feet to the concrete slab below.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about August 19, 2021, and times prior thereto, employees were removing metal ceiling panels from an existing metal roof without the use of fall protection, exposing the employees to the hazard of falling approximately 17-feet to the concrete slab below.

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. If your establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.   On or about August 30, 2021, the employer failed to electronically submit injury and illness data for calendar year 2020 from their OSHA Form 300A or equivalent form by March 2, 2021. The establishment employed 57 employees and is classified as 238310-Drywall and Insulation Contractors.
