29 CFR  1910.303(e)(1)(ii): Electrical equipment was used when the markings giving voltage, current, wattage, or other ratings as necessary were missing:  On or about August 23, 2021, the employer did not ensure electrical equipment was marked with the voltage, current, wattage, and other ratings, thereby exposing employees to an electrical hazard while working on an metal enclosed electrical cabinet located in the US 4 Substation. The employee was attempting to determine the cause of an electrical outage at the facility building "B" and was testing fuses.

29 CFR  1910.303(h)(5)(iii)(A): All entrances were not kept locked unless they were under the observation of a qualified person at all times:  On or about August 23, 2021, the employer did not ensure all entrances to high voltage electrical rooms over 600 volts were locked and only accessible to qualified employees, in that, the entrance to the high voltage electrical room containing three phase primary voltage (4KV), located in Building B, US 4 Penthouse substation, was not marked and the room was not secured, exposing employees to an electrical hazard while storing material in the room. Unauthorized employees were allowed to access the room and the room was used for overflow storage including filters and other cleaning supplies.

The entrance to the high voltage electrical room containing three phase primary voltage (4KV), located in Building B, US 4 Penthouse substation, was not marked with proper signage, exposing employees to an electrical hazard while storing material in the room.

29 CFR  1910.332(b)(3): Qualified persons (i.e. those permitted to work on or near exposed energized parts) were not, at a minimum, trained in and familiar with the following: (i) The skills and techniques necessary to distinguish exposed live parts from other parts of electric equipment, (ii) The skills and techniques necessary to determine the nominal voltage of exposed live parts, and (iii) The clearance distances specified in 1910.333(c) and the corresponding voltages to which the qualified person will be exposed.   On or about August 23, 2021, the employer did not ensure employees were adequately trained in the safety-related work practices to be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. Specifically employees were not trained in electrical work for voltages greater than 600 volts.
