On or about August 26, 2021, the employer did not ensure that employees were protected from the point of operation on Johnford SV-32 CNC Machine Center #1 with Fanuc 18M Controller in that an interlock device that was present was not functional, permitting the doors of the machine to be opened during the machine cycle.   b.	On or about August 26, 2021, the employer did not ensure that employees were protected from the point of operation on Johnford SV-32 CNC Machine Center #2 in that an interlock was present, but its key was not bolted to the door, allowing the key to remain in the interlock when opening, in turn permitting the doors of the machine to be opened during the machine cycle.   c.	On or about August 26, 2021, the employer did not ensure that employees were protected from the point of operation on Johnford SV-40 Machine Center #5 (sn: MLB190646) with Fanuc Oi-MD controller in that an interlock device that was present was not functional, permitting the doors of the machine to be opened during the machine cycle.  d.	On or about August 26, 2021, the employer did not ensure that employees were protected from the point of operation on Johnford SV-40 Machine Center #6 (sn: MLB12524) with Fanuc Oi-MD controller in that an interlock device that was present was not functional, permitting the doors of the machine to be opened during the machine cycle.
