29 CFR 1910.134(m)(2)(i): The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the information required by 29 CFR 1910.134(m)(2)(i)(A) through (m)(2)(i)(E): (a) Chapel Manor located at 1104 Welsh Road Philadelphia, PA: The employer did not ensure that fit test records included all of the required information such as but not limited to the type of the fit test performed, and model and style of the respirator used for fit testing. The employees were required to wear N95 filtering facepiece respirators such as but not limited to 3M 8210, Makrite Sekura (TC-84A-7638), and Cardinal Health (TC-84A-4107) while providing care to suspected and confirmed positive COVID-19 residents. Condition noted on or about August 25, 2021.
