DeBakey VA Medical Center located at 2002 Holcombe Blvd., Houston, TX 77030: a) On or about August 17, 2021, an employee was hospitalized due to contracting COVID-19 in the workplace. The employer did not report the in-patient hospitalization to OSHA within twenty-four hours. The in-patient hospitalization was reported to OSHA on August 21, 2021. b) On or about August 10, 2021, an employee was hospitalized due to contracting COVID-19 in the workplace. The employer did not report the in-patient hospitalization to OSHA within twenty-four hours. The in-patient hospitalization was reported to OSHA on August 21, 2021. c) On or about August 28, 2021, an employee was hospitalized due to contracting COVID-19 in the workplace.
