29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Location - 2 Blueberry Lane, Burlington, MA On or about 08/07/2021, employees were exposed to a fall hazard while working on a steep roof without fall protection. JPFM Construction LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926 501(b)(13), which was contained in OSHA inspection number 1389021, citation number 1, item number 4a and was affirmed as a final order on 05/06/19, with respect to a workplace located at 362 Boston Road, Billerica, MA.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top. 362 Boston Road, Billerica, MA 01821 On or about March 27, 2019, employees were exposed to falls while accessing a the roof where the ladder side rails did not extend a least 3 feet above the roofs eve. JPFM Construction LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29CFR1926.1053(b)(1), which was contained in OSHA inspection number 1389021, citation number 1, item number 6a and was affirmed as a final order on 05/06/19, with respect to a workplace located at 362 Boston Road Billerica, MA.
