29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 8/26/21 employees were exposed to a fall of approximately 17 feet while performing roofing activities on a residential home located at 596 Vinita, Akron, Ohio 44320.  The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.    Kirby Vanfossen was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1520757, citation number 2, item number 1 and was affirmed as a final order on (5/3/21), with respect to a workplace located at 2377 Medina Road, Medina, Ohio 44256.  Kirby Vanfossen was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1486208, citation number 1, item number 2 and was affirmed as a final order on (10/15/20), with respect to a workplace located at 635 Canal Street, Canal

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a)  On or about 8/26/21 employees were exposed to a fall of approximately 17 feet while performing roofing activities on a residential home located at 596 Vinita, Akron, Ohio 44320.  The employees were using two extension ladders to access the roof and the employer did not ensure that they extended at least three feet above the landing area.   Kirby Vanfossen was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1486208, citation number 1, item number 3 and was affirmed as a final order on (10/15/20), with respect to a workplace located at 635 Canal Street, Canal Fulton, Ohio 44614.
