29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Location: basement On or about: 8/26/21  a) An employee, who was working below overhead hazards such as, but not limited to a falling 4" metal coupler, was not wearing a hard hat.    NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR  1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired:    location: basement on or about: 8/26/21  a) An employee was working from a defective Skyjack SJIII 3219 scissor lift. The control panel  above the work platform did not allow the operator to raise the lift. A second employee had to raise the platform using a switch at the bottom of the lift. The lift was not immediately repaired, replaced or removed from service until repaired.     NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  location: basement on or about: 8/26/21  a) An employee, engaged in electrical work, was working from a Skylift scissor lift without an adequate guardrail system in place. The midrail at the access area was not engaged. The employee was exposed to a fall of 11 feet 8 inches.    NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
