29 CFR  1910.133(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  ANODIZE AREA #1:  On or about August 27, 2021, employees dipping parts into floor-level anodize baths using a hoist-assist to move parts in and out of plating tanks wore safety glasses rather than splash goggles.  Some of the tanks contained acidic solutions and most of the tanks were maintained at a temperature between 140 and 212 degrees F.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about August 30, 2021, two relocatable power taps (RPTs) in the Conference Room used to energize multiple desktop computer systems were connected in series.  According to the UL listing for RPTs, the devices are to be directly connected to a permanently installed branch circuit receptacle and are not to be series-connected to other RPTs or connected to extension cords.

29 CFR  1910.303(b)(7)(iv): There were damaged parts that adversely affected safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:  SILVER PLATING AREA #6:  On or about September 23, 2021, an electrical outlet located between tanks 14 and 15 was separated from the conduit due to rust and corrosion of the outlet box thereby exposing live wires to potential damage and exposure to plating solutions.  Employees were exposed to potential for electric shock and/or burns.

29 CFR  1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  SILVER PLATING AREA #6:  On or about September 23, 2021, the employer did not ensure that electrical conductors were protected from abrasion or other damage in that a metal electrical outlet intended to be wall-mounted was not properly installed.  The outlet corroded and broke away from the conduit it was attached to exposing electrical conductors that powered the outlet.  The outlet was hanging by the conductors.

29 CFR  1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:  RACKING AREA JUST OUTSIDE THE MAIN PLATING AREA:  On or about September 23, 2021, two electrical panels were located with 22 inches of a box dryer.  One panel, PP2, had 240V service.  The other, P2, had 120V service.
