On or about August 27, 2021, at jobsite located in 111 N. Berteau Ave., Elmhurst, IL 60126, employees were exposed to eye injuries while installing shingles using the nail gun. Eye protection was not being utilized while employees engaged in this activity. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: On or about August 27, 2021, at jobsite located in 111 N. Berteau Ave., Elmhurst, IL 60126, employees performing roofing work at a new residential property were exposed to fall hazards in excess of 20 feet, with no fall protection when the employer failed to ensure they were adequately trained to recognize and minimize the hazards associated with fall. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
