29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Instance a. Hendey Lathe - On or about August 25, 2021, an employee received a severe amputation and hand injury while performing sanding procedures where machine guarding was not utilized to prevent the hazard. Instance b. On or about August 26, 2021, the Hendey Lathe machine was not adequately guarded where the lead screw guarding was not installed to protect employees from hazards associated with rotating parts. Instance c. On or about August 26, 2021, the Clausing-Colchester 15" Lathe machine was not adequately guarded where the lead screw guarding was not installed to protect employees from hazards associated with rotating parts Instance d. On or about August 26, 2021, the Logan (model 6560) Lathe machine was not adequately guarded where the lead screw guarding was not installed to protect employees from hazards associated with rotating parts
