29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Jobsite, Morgan Lane, Monaca, PA 15061: On or about August 30, 2021, during excavation activities, the employer did not ensure that an adequate cave in protective system was in place, in there was no cave in protection inside the trench that was measured to be 8 feet in depth where employees were working.
