Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.( 3 Phillips St., Boston, MA On or about, August 30, 2021, employees were exposed to a six-story fall to the ground while removing and installing a rubber roof system on a flat roof without fall protection. On August 30, 2021, a worker performing roof work work fell off a six-story roof to the ground.
