29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  3 Phillips St., Boston, MA On or about, August 30, 2021, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury related to fall hazards related to low roofing work, flammable liquid fire prevention and fire suppression, roof chemical physical and health hazards, including lead flashing, power tools , temporary flexible cords, and hand wheel hoist erections and placement.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  3 Phillips St., Boston, MA On or about August 30, 2021 employees were removing and installing a rubber roof system on a flat roof where the employer did not provide a training program for each employee exposed to fall hazards to  recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  3 Phillips St., Boston, MA On or about, August 30, 2021, employees were exposed to a six-story fall to the ground while removing and installing a rubber roof system on a flat roof without fall protection.  On August 30, 2021, a worker performing roof work fell off a six-story roof to the ground.
