(a) Location - Lahey Clinic, 41 Mall Road, Burlington, MA 5th Floor mechanical room On or about 8/23/2021, adequate training in the hazardous energy control procedure was not provided to an authorized employee in that the employee failed to wait for the belt to stop before servicing an air handling unit.
