29 CFR  1926.20(b)(2): Frequent and regular inspections of the job sites, materials, and equipment were not made by a competent person(s) designated by the employer:  a. Story Renovations, LLC at 1275 N Santa Fe Drive, Denver, CO 80204: On and before August 30, 2021, the controlling employer did not ensure that three employees of Mowers Concrete Construction LLC., subcontractor, were protected from injury, in that employees were conducting formwork at heights without the use of a fall protection system. This condition exposes to employees to serious head injuries, broken bones and death from falls.    b. Story Renovations, LLC at 1275 N Santa Fe Drive, Denver, CO 80204: On and before August 30, 2021, the controlling employer did not ensure that employees of  subcontractors working in adjacent areas were protected from injury, in that an employee of West Drilling LLC. was operating a Bobcat T550 Compact Track Loader and the signal alarm on was not operational when the equipment was use. This condition exposed employees to struck-by hazards.  c. Story Renovations, LLC at 1275 N Santa Fe Drive, Denver, CO 80204: On and before August 30, 2021, the controlling employer did not ensure that employees of subcontractors working in adjacent areas were protected from injury in that employees  worked and walked around unguarded reinforcing steel protruding from concrete while performing work operations. This condition exposed employees to impalements.  d. Story Renovations, LLC at 1275 N Santa Fe Drive, Denver, CO 80204: On and before August 30, 2021, the controlling employer did not ensure that three subcontracted employees of Mowers Concrete Construction LLC., subcontractor, were protected from injury in that the working platform on the cantilevered scaffold was not fully planked and there was a gap between the planks that ran the whole length of the scaffold. This condition exposed employees to broken bones and head injuries.
