Mowers Concrete Construction LLC., at 1275 N Santa Fe Drive, Denver, CO 80204: On and before August 30, 2021, as the exposing, creating and correcting employer did not ensure the working platform on the cantilevered scaffold was fully planked in that there was a large gap between the planks on which a worker was standing that ran the length of the scaffold. This condition exposed employees to falls.

Mowers Concrete Construction LLC., at 1275 N Santa Fe Drive, Denver, CO 80204: On or before August 30, 2021, as the exposing, creating and correcting employer, did not ensure employees were protected from injury in that each personal fall arrest system was not inspected prior to use and that defective components were not removed from service. Employees were using personal fall arrest systems with frayed harnesses and lanyards. This condition exposed employees to broken bones and head injuries.

This condition exposed employees to impalements.
