Test voltages and the maximum intervals between tests were not in accordance with Table E-4 and Table E-5. Location: site on or about 8/31/21, The employer did not ensure the rubber insulating sleeves used by an employee had been tested within the limits of table 5. The sleeves had a stamped test date which was just over two years prior to this current use.
