29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition Warehouse:: 1) Protruding rebar near one stairway of the mezzanine created a tripping hazard. 2) Damaged flooring in aisle 3-H and aisle 14-C were not repaired creating a tripping hazard. 3) Raised sills where sections of buildings meet created a tripping hazard. 4) A pipe trench at the end aisle at the receiving doors was inadequately covered creating a trip hazard.
