29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a) Gen 4 Construction, LLC at the address of 1028 North Main Street in Bowling Green, Ohio  On or about September 1, 2021 the employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness. Employees were exposed to hazards associated with falls of approximately fifteen feet to an asphalt covered ground.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who might be exposed to fall hazards:  a) Gen 4 Construction, LLC at the address of 1028 North Main Street in Bowling Green, Ohio  On or about September 1, 2021 the employer did not ensure employees had been trained in the appropriate fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures. Employees engaged in roofing activities were exposed to hazards associated with falls of approximately fifteen feet to an asphalt covered ground.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  a) Gen 4 Construction, LLC at the address of 1028 North Main Street in Bowling Green, Ohio  On or about September 1, 2021 the employer did not ensure employees were protected from falls while performing roofing activities atop a low slope commercial structure. Employees engaged in roofing activities were exposed to hazards associated with falls of approximately fifteen feet to an asphalt covered ground.
