(b) At aisle 419/420, on or about August 31, 2021, a sit-down fork lift operator was observed exiting the aisle without sounding his horn exposing workers to struck-by hazards where he had not been retrained since 2014.  (c) At bay 18, On or about August 31, 2021, a sit-down forklift operator was observed not looking in the direction of travel 100% with workers on foot in the immediate area exposing them to struck-by hazards where the operator had not been retrained since 2014.
