29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: On or about September 1, 2021, at an excavation site located at 14640 Hertz Parkway in Oklahoma City, OK, no protective devices were in place to protect an employee working in a trench that was more than 5 feet in depth. The employee was exposed to the hazard of being crushed by fall soil.
