On or about September 1, 2021, and at times prior thereto, an employee was installing shingles to the roof of the residential home under new construction up to approximately 18 feet high without fall protection training, exposing the employee to a fall hazard.

On or about September 1, 2021, two employees accessed the roof of a two-story home by using portable ladders that were not extended at least 3 feet above the upper landing surface, exposing the employees to a fall hazard of approximately 12 feet to the ground below.
