29 CFR 1926.102(a)(1):The employer did not ensure that each affected workers was using eye or face protection when exposed to eye or face hazards from flying particles. On or about September 1, 2021, two workers were using a pneumatic nail gun to secure shingles to a roof without wearing appropriate eye protection, exposing the workers to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(13):Each worker engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were workers provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about September 1, 2021, and at times prior thereto, two workers were installing shingles to the roof of the residential home under new construction up to approximately 18 feet high without fall protection, exposing the workers to a fall hazard.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about September 1, 2021, two workers accessed the roof of a two-story home by using portable ladders that were not extended at least 3 feet above the upper landing surface, exposing the workers to a fall hazard of approximately 12 feet to the ground below.
