1926.21(b)(2) The employer did not adequately instruct employees in the recognition and avoidance of unsafe conditions in that ironworker employees were exposed to struck-by and/or crushing hazards while assembling and connecting unsecured steel ductwork frame supports that were approximately 12' long, 65" high, and weighed approximately 500 lbs. On or about August 24, 2021, iron worker employees were exposed to struck-by and/or crushing hazards while assembling and connecting steel ductwork frames supports that were approximately 12' long, 65" high, and weighed approximately 500lbs. The frames were not adequately secured to prevent tipping, sliding, and/or falling, while employees were manually assembling the frame beams, columns, cross braces and placing and tightening the frame(s) connecting bolts.
