Employees engaged in excavation/trenching activities while installing a new water main connection to the existing infrastructure where no inspections were completed by a competent person. The trench was approximately 5 feet deep, 40 feet long and 5 feet wide.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance paragraph (b) or (c) of 29 CFR 1926.652. a) West Walnut Street between South State Street and Thorn Street South bound lane closure, Magnolia, DE 19962: On or about 09/02/2021, the employer failed to protect employees working in a trench by an adequate protective system. Employees engaged in trenching and excavating work adding a new water line. The competent person required employees to enter and work in a trench without any type shielding system as required by OSHA standards. The competent person was not able to clarify what type of soil was at the site so they could be properly prepared with the required shielding structure.
