29 CFR 1910.133(a)(1):Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment: a.) UPMC Horizon dba MERP, Sanitation area of the facility, on or about September 2nd, 2021 and times prior thereto: Employees worked with and around corrosive chemicals including, but not limited to, Avert brand Sporicidal Disinfectant Cleaner during the sanitation of medical equipment. An employee that performed work operations with these chemicals did not utilize appropriate eye protection. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a.) UPMC Horizon dba MERP, Sanitation area of the facility, on or about September 2nd, 2021 and times prior thereto: Employees worked with and around corrosive chemicals including, but not limited to, Diversey brand Avert Sporicidal Disinfectant Cleaner during the sanitation of medical equipment. Employees did not have access within the immediate work area to a suitable eyewash station. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

The employer did not make their Hazard Communication Program available upon request in accordance with 29 CFR 1910.1020 (e).
